When you need to dispose of chemical, biological or radioactive materials through EHS, you must submit
a request. Currently, there are two options: Electronic Submission or Paper-based Submission. For either option, you must select
the appropriate Pick Up Request Form (PURF) for your material. Please read the descriptions below to determine which PURF to use.
Electronic Submission
For the collection of all unwanted materials - this system requires authorization to access, with the appropriate Authorized User's
or Registered User's PawPrint and his/her EHS password. Once you are logged in, the system will be populated with data appropriate to
your registration.
Note to AUs and RUs: If you have not accessed a similar EHS system in the past, you will need your Employee ID number
to access this system. If you have accessed the system and forgot your EHS password, follow the provided link. Be sure to choose
an EHS password that is different from your University (PawPrint) Password so you can share it with your workers if
you so choose. Follow this link to find your Employee ID number if needed.
Note to Workers: this system uses the PawPrint of the Authorized User (AU) or Registered User (RU). If you are
not an AU or RU, your PawPrint will not work. Contact your AU or RU for his/her PawPrint and EHS Password.

Paper-Based Submission
Used Materials
For collection of unwanted materials, including used chemicals, unknowns, unused chemicals no longer
in the original bottle, and biological/infectious materials, use the PURF and instructions below. These items should have a yellow
Hazardous Materials Label (HML) on the container.
