This tutorial is intended to help users submit a RAM waste pickup request in EHSA. RAM waste containers are required to be picked up within 12 months of their open date.

Submitting a Dosimetry Application

STEPS

1. To begin...
   1. Click here for a shortcut to the form and login to EHSA or

   2. Login to EHSA and click on EHS Assessment Click on +Browse New Forms at the top left corner to open the Assessment Form window. Browse for the Dosimetry Application form under Radioactive Material Use Forms.

      

2. The first tab of the application summarizes the prerequisites that must be completed prior to application and provides a link to a video tutorial on how to complete the application as well.
3. The comment box may be used to provide specific information such as dosimeter wear period for temporary workers. Future deactivation dates can only be set within 5 months of the dosimeter wear date.

4. Click on the next tab, Assessment Questions, to complete the form. Be sure to complete all fields.

   

5. The form will expand and ask for the Dosimetry Coordinator and Subaccount Code. Dosimetry coordinators are listed by their last name then first name (e.g. Doe, Jane). Subaccount Codes are typically compromised of two or three letters (e.g. CT, ATT, RYX...)

6. To submit the application, apply your e-signature by clicking on the green Sign button and press Save as Complete.

   

7. You have now submitted a dosimetry application! Your dosimetry coordinator and radiation safety will review your application. If your dosimeter is approved, you should receive it in 3 - 10 days.
